Tried the search function but didn't see much aid so . . .
since thumb notches are a chargeable option - what models[if any] do the notches come as part of the 'base' model?

Model 5 for sure. Having said that I have one or
two older 5's that have no thumb notches.

Add 6-4 1/2's, Stanaback Specials, AGRussell specials, Tailors and Triathletes...and, (but don't tell anyone), we now put them on 14 and 15 CDT's with single hilts as standard....

TN aren't standard on a Stanaback Secial. I almost always order them but they are charged. (Unless something's changed?)
