The top paracord is intended for several purposes / all of them related to attaching the sheath in various ways to a variety of equipment. Shoulder hook-up, inverted on a jump harness or MaeWest are just a couple of them. Ben was also correct in mentioning a pack...but also a pack "horse" or just about anything else you can conjour up!
I have a #18-5 1/2" that I rig up on a small day pack (Used mostly at night, actually) when trailing game. It mounts on my left shoulder, inverted and ready to go should I need it for "God knows what". Track a big boar at night and you'll figure it out! Thi rig allows me to carry my Glock 10mm on my right hip and neither interfere with each other.
